Initial Guidance Meeting – $500 flat fee, nonrefundable. This 2-hour session includes a comprehensive review of your loved one’s medical, financial, and safety needs, a clear care-roadmap with next steps, and a follow-up phone call in case your needs change. Ideal if you’re just starting the elder care journey and need expert guidance to make confident decisions.
One-time detailed consultation.
($500 fee will be applied to your Medicaid Roadmap or Navigator Plan fees.)

Medicaid Roadmap
$2,000 – $3,000
Pricing depends on case complexity and state requirements. You’ll receive a clear range before work begins.
Comprehensive Medicaid application support, including gathering financial records, completing all forms, coordinating signatures, and submitting documentation to the appropriate agencies. Ideal for families who want expert help managing every detail of the application and follow-up requests.

Navigator Plan
Ongoing Care Navigation Support – all for $3,500 per year. This plan provides check-in calls, priority email support for questions as they arise, and coordination help when care needs change (such as new diagnoses, hospital discharges, or moves to assisted living). Families use this option when they want a trusted guide walking beside them throughout the process, not just a one-time consultation.

What To Expect In Sessions
In our meeting, we’ll walk through your key financial and planning documents, including income and asset summaries, recent bank and investment statements, long‑term care or health insurance policies, powers of attorney, advance directives, wills or trusts, and any existing care or discharge plans. Reviewing these together helps us understand your family’s situation and outline clear next steps.
At Elder Care Navigator, we listen first, then design clear, practical plans for your loved one’s care. We translate medical jargon, coordinate resources, and stand beside your family so every decision feels informed, compassionate, and manageable.
